Services for the whole family

  • Not feeling well and need a consultation? We provide high-quality, evidence-based evaluations, diagnoses, and treatments for all our patients.

  • Review your full family and personal medical history, current medications, immunizations, and discuss any health concerns. Determine what health issues you may be at risk for and make a plan to prevent them. Wellness visits are an integral part of our prevention model.

  • Were you recently treated for an illness, had labs drawn during your wellness visit, or have follow-up questions or concerns to discuss? We would love to see you to follow-up on your labs, recent diagnoses, and treatment plans.

  • Can’t make it to the clinic or prefer accessing our holistic services from the comfort of your home? We are committed to supporting you by making our services accessible to you virtually. Our video and phone consultations can include recommendations and/or monitoring of your health and wellbeing. An in-person visit may be requested if we feel that a physical assessment would be beneficial.

  • Your emotional and mental wellbeing are just as important as your physical health. Counseling sessions can be in-clinic or virtually, depending on your preference. We take patient confidentiality very seriously and nothing is ever recorded or stored during our sessions. Counseling is with our Primary Care Provider. Please be advised that we may refer you to a psychotherapy specialist if we deem it necessary.

WOMEN’S HEALTH

Specialized Services

  • We believe in the Nurse Practitioner, Midwifery, and Public Health models of care; that is, disease prevention, health promotion, and supporting normal physiological changes throughout a woman’s lifetime. During your annual wellness visit, your health care provider will review your past medical, gynecological, obstetrical, family, and social history, perform a thorough physical examination and make evidence-based recommendations according to your risk factors.

  • Talk with our women’s health specialist about your menstrual and sexual health, or any problems you may be having (such as abnormal bleeding or pain). You will have a full physical assessment and evidence-based diagnosis and treatment plan.

MATERNAL HEALTH

Support Throughout Your Pregnancy Journey

  • Did you know that your journey to a healthy pregnancy begins before you even conceive? Many women do not know they are pregnant until after their 8th week of pregnancy, when most of the baby’s organs would have formed already. Preconception care is vital to maximize your health prior to conceiving, increasing the likelihood of having the healthiest pregnancy possible.

    What to expect during your preconception visits:

    • Meet with medical provider, specialized in Women’s Health and Midwifery

    • Review full medical, surgical, family, vaccination, social histories, and current medications

    • Comprehensive physical examination and necessary labs will be taken

    • Discuss steps to encourage a healthier lifestyle and elimination of toxins and harmful substances from your life in order to protect your baby

    • Fertility awareness discussion (if necessary)

  • The health of the intrauterine environment throughout pregnancy may impact the long-term health of the baby. Our team is dedicated to ensuring good health for you and your baby’s future.

    What to expect during your prenatal visits:

    • Work hand-in-hand with our Women’s Health Nurse Practitioner & Certified Nurse Midwife to ensure that you and your baby remain healthy

    • Counselling regarding healthy nutrition and lifestyle, minimizing stress and eliminating exposure to chemicals and toxins

    • Regular assessments you to make sure that your baby is developing well

    • Labs drawn as a part of your health assessment in collaboration with our partner laboratory

  • Did you know that your body goes through a lot of physiological and psychological changes even after you give birth? We believe in the importance of postpartum care and seeing our Mamas and babies during their first week after birth, at 2 weeks, 6 weeks, and 6 months postpartum.

    What to expect during your postpartum visit:

    • Assessment during critical period, known as the “fourth trimester”

    • Discuss changes you are going through and how you and your baby are transitioning after birth

    • Review labor and birth history and current health status

    • Receive comprehensive postpartum physical examination and obtain any necessary labs

    • Discuss recommendations to optimize you and your baby’s health and wellbeing

    • Guidance regarding breastfeeding, parenting, reproductive life plans, and how to identify warning signs and symptoms

NEWBORN & PEDIATRICS

  • We are committed to making sure that all babies transition into life smoothly.

    • Meet with clinician certified in Newborn & Family Practice

    • Newborn visits should occur during the first week after birth, and at the age of 2 weeks, 6 weeks, 3 months, 4 months, 6 months, 9 months, 12 months, 15 months, and 2-years

    • Review developmental milestones

    • Thorough physical examination and guidance regarding age-appropriate expectations

    • Address recommended vaccine schedule and questions or concerns parents may have
